New forms for 2010 now available!
To sponsor a Catepillar or Serve on a team, download and complete the applicable form.
Please remember that ALL forms must be signed by the applicant's Minister or Pastor!
Before a Catepillar's name can be entered into the computer, for a Flight, we need the original forms, the deposit and the motivational letter.

**PLEASE NOTE:** There are pages that are in the Catepillar applications forms that are FOR SPONSORS ONLY and it is important that these are not given to the Catepillar!
